Screening forms are often distributed to patients to better understand their needs, goals, and wants. However, it can be difficult for some patients to complete screening forms through online patient portals because of barriers with using technology, or before visits because they have yet to fully understand their diagnosis. Without good identification of patient goals early on in patient care, the treatment plan may not be fully effective or be subject to unanticipated setbacks.

The Pancreas Multidisciplinary Clinic (PMDC) team hopes to develop a multimodal method of screening to equally care for all patients presenting to the PMDC. They are optimizing a screening form to focus on goals of care, palliative care, nutrition, and pancreas enzyme need (IRB00368874). When these questions are optimized, they hope to use IHM funding to create and deploy an electronic tablet version for patients to complete at their visit if they are unable to complete it prior via MyChart. With this option, there is an opportunity for in-person interaction between patients, caregivers, and staff members to clarify, educate, and refer.
